U.S. Passport Information and Application

Appointments are required for all services at both office locations.

To Obtain a U.S. Passport Book/Card, the Following Items Are Required:

  • All Applicants Present
    • Including all minors regardless of age.
  • Completed Application (FORM DS-11)
    • Completed in black ink only.
    • No cross-outs, correction fluid, or tape allowed, prepare new form.
  • Proof of OF U.S. Citizenship (Original Documents)
    • Certified Birth Certificate – or –
    • Certificate of Naturalization or Citizenship – or –
    • Previous U.S. Passport.
  • Proof of Identity
    • (2) Forms of Identification (Driver’s License preferred, plus additional photo ID).
  • Photograph
    • Current (within 6 months) Passport Photo.
    • Can be done at our offices for $15.00.
  • Passport Fees to U.S. Department of State
  • Processing Fees to Douglas County
    • Check, Cash, or Credit/Debit Card per passport processing for Douglas County.

Additional Information:

  • Both parents and minor child must be present when applying for a passport for a minor under the age of 16. If one parent is unable to be present, they may complete Form DS-3053 (Statement of Consent or Special Circumstances/ Issuance of a Passport for Minor Under Age 16).
  • One parent and child should be present for child aged 16-17.
  • If a child 15 years or younger has a previous passport, BOTH the previous passport and the certified birth certificate are needed.
  • Passport Photo Service available at both office locations for $15.00 per photo.
  • Express Mail option available for $33.25 to expedite shipping to the U.S. Dept. of State.

Renewal of Passport Book and/or Card:

  • Review Form DS-82 (U.S. Passport Renewal Application for Eligible Individuals)
    • If able to answer “yes” to questions as stated on application, then follow steps to renew on your own.
    • Application and previous passport book mailed on your own. Our office can review application for completeness and provide photo
  • If unable to answer “yes" to questions as stated, then follow steps for NEW passport.
